NCQA continues to make changes and improvements with their product for your primary care practice to utilize and demonstrate that you are patient centered. Take time to review these changes and make sure your practice is able to proceed. Additionally, we will explore how to get started with the Joint Commission Ambulatory Care PCMH Add-On accreditation process. This session is for practices with NCQA PCMH recognition as well as for those that have not pursued recognition with NCQA or accreditation with the Joint Commission.
1. Review the updates to the NCQA PCMH version 5 standards and criteria as well as the changes made to Q-PASS.
2. Identify the NCQA PCMH Annual Reporting 2020 criteria that may take more time and updated requirements with a focus on the Social Determinants of Health.
3. Understand how a practice can add the NCQA Behavioral Health with distinction option and if applicable enroll in School Based Medical Home, Patient Centered Specialty Practice and Patient-Centered Connected Care.
4. Review the general Joint Commission Ambulatory Care PCMH Add-On accreditation process.
